I agree for Mazda, its affiliates, or its authorized dealers to:

1. Collect, use, disclose, and process my personal information set out in this form and/or otherwise provided by me or possessed by Mazda, its affiliates, or authorized dealers for the following purposes:
   • Sending marketing, advertising, and promotional information about other products or services that Mazda may be offering, conducting surveys for the vehicle(s) purchased and services performed by way of postal mail, fax, SMS, IG, Facebook Messenger, Viber, Whatsapp, Line, Telegram by way of voice call/phone call, and/or electronic transmission to my email address.
   • Conducting feedback and comments for vehicle(s) purchased and services performed;
   • Others.
2. Retain my information for a period of five (5) years from the date of purchase of my vehicle or at such time that I submit to Mazda, its affiliates, or its authorized dealers a written cancellation of this consent, whichever comes first. I agree that my personal information will be deleted/destroyed after this period;
3. Retain my personal information in Mazda, its affiliates, and authorized dealers’ database shared with third-party service providers; and
4. Share my personal information to third-party service providers or its agents (including lawyers/law firms) for any legitimate business purpose. I am assured that security systems are employed to protect my information.

I warrant that I have read and understood all of the above provisions and I hold free and harmless and indemnify Mazda, its affiliates, or authorized dealers from any complaint, suit, or damages, which any party may file or claim in relation to my consent.
